Faculty Share Knowledge at National Meetings

Houston Methodist Department of Otolaryngology – Head and Neck Surgery faculty members spent the spring and summer presenting to contemporaries in Las Vegas, Nashville and Washington, DC, on current research, new strategies for treating patients and some basics about office procedures, insurance questions and how to best serve a range of patients. More than 40 presentations included faculty panel discussions, moderation by faculty members, poster and podium presentations and published articles, many of which have links included below.

Takashima presents masterclass in frontal sinus dissection
Mas Takashima MD, FACS, Professor and Chair, Otolaryngology – Head and Neck Surgery at Houston Methodist Hospital was selected to lead a “Masterclass in Frontal Sinus Dissection Using 3D-Printed Simulation Models” at the AAO-HNSF (American Academy of Otolaryngology – Head and Neck Surgery and Foundation conference in Nashville. The class was unique in that it was more than a lecture. It also included hands-on training for the attendees with 3D-printed simulation models. The frontal sinus is considered the most challenging sinus area to access. After an initial lecture, all the attendees went into a dissection arena for actual training. The latest technology was available including 3D advanced models where experience with real bone and tissue was simulated. High tech equipment included drills and image-guiding system.

Caregivers improve recovery rate
In a recent study of the role of caregivers in head and neck cancer patients who undergo surgery, researchers from Houston Methodist’s Department of Otolaryngology – Head and Neck Surgery found that having a caregiver was associated with reduced length of hospital stay as well as fewer patients who experienced recurrence of the disease. Daniel Gorelik, MD, MPH, presented the results at the American Academy of Otolaryngology meeting in Nashville in September. The research team also included Aatin K. Dhanda, BA; Najm S. Khan, MBS, Nadia Mohyuddin, MD, FACS, Joshua Kain, MD, and Laura Kim, MD, MSc. Gorelik is a new resident of the department after serving as a clinical research fellow last year. Gorelik said the study was conducted to raise awareness of the importance of caregivers and to enable early engagement of additional resources for the patient. He added that further studies are needed to strengthen the results. A total of 91 patients with caregivers were studied and 13 who had no caregiver. The average length of the patients’ hospital stay was 19.8 days for those without a caregiver and 11.6 for patients with a caregiver.

Dry mouth is increasing in the United States
The prevalence of xerostomia, or dry mouth, is increasing in the United States and bringing with it concerns associated with morbidity and quality of life, according to a recent study presented by Daniel Gorelik, MD, MPH, David Z. Allen, MD, and Nadia Mohyuddin, MD, at the American Head and Neck Society 11th Annual Meeting. A total of 2,290 (6.9%) individuals were identified as having xerostomia in a cross-sectional analysis using data from the National Health and Nutrition Examination Survey (NHANES). Adults ages 18-85 were assessed via questionnaire data to determine xerostomia prevalence over two time periods,1999-2004 and 2011-2014. Mean age was 56.6 (3.04%) years, and 49% patients were male. Patients with xerostomia in the overall population were found to be associated with female gender, older age and lower household income. Several comorbid conditions were associated with higher likelihood of developing persistent xerostomia including medication use, presence of thyroid disease and smoking status. Individuals regularly seeking dental care and the perception of excellent teeth and gum health was associated with a lower likelihood of having xerostomia. Patients with poor dental health were twice as likely to have xerostomia. Patients who reported any medication use were roughly five times as likely to report xerostomia.

Challenge of postnasal drip and chronic cough included in study with favorable results
Postnasal drip (PND) and chronic cough (CC) have long pervaded the Southeast Texas community and many other areas of the United States where the population suffers from allergies and asthma. Unfortunately, these conditions are more alarming since the initial days of COVID-19. Coughers can be ostracized in public by communities who are more concerned about germs than ever. Thus, physician-scientists from Houston Methodist conducted a study of chronic rhinitis and determined the inclusion of PND and CC in a patient assessment instrument could provide important additional information for the characterization of the disease and outcomes after any therapeutic treatment. Combined data from 228 patients (57.9% women, 42.1% men) were studied from 35 unique study centers in the United States and Germany. Temperature-controlled radiofrequency (TCRF) neurolysis of the posterior nasal nerve was determined to improve chronic symptoms as measured by the reflective total nasal symptom score, including rhinorrhea, congestion, nasal itching and sneezing. The study was published recently in the International Forum of Allergy & Rhinology and was conducted by Daniel Gorelik, MD, MPH, Jumah G. Ahmad, MD, Samuel E. Razmi BS, Masayoshi Takashima, MD, Yin Yiu, MD, Apurva Thekdi, MD, Murugappan Ramanathan, MD, Aatin K. Dhanda BA, Michael T. Yim, MD, and Omar G. Ahmed, MD. The study concludes that validation of an expanded assessment instrument is needed to better understand the results of including postnasal drip and chronic cough in a study of this challenging condition for patients with CRS and other applicable diseases.
